The ”Memory Symptom Questionnaire for Working-age Adults” form has been developed
in the project ”Cerebral Health and Memory of Working-age Adults” (Aivoterveys ja muisti
työikäisillä) as a tool for those who work in patient reception in, for example, occupational
health care.

Memory Symptom Questionnaire for Working-age Adults
Target group
Memory Symptom Questionnaire for Working-age Adults is a form that can be used by professional practitioners serving working-age adults, such as occupational health and public health nurse practitioners and professionals in the field of substance abuse and mental health. Completing the form does not require training; a thorough familiarisation with its content is sufficient.
How does the concept work in practice?
The form can be used in many different ways. It is recommended that anyone attending an occupational health check is asked at least the first three questions. If problems are reported, a further interview on the symptoms and when they started is carried out.
The second page of the form serves as a checklist of all the factors that need to be assessed when investigating the underlying cause of memory problems. If necessary, the interviewer can refer the customer to further neurological examination based on the information gathered on the form.

How does the concept help people with memory disease or prevent memory disease?
The form encourages health practitioners to raise the issue of memory and serves as a checklist when investigating the underlying cause of memory problems. According to the feedback, the form also makes it easier to make referrals to further tests. If memory problems are raised more systematically among the working-age group, access to further tests and treatments can be speeded up.

Practitioners serving working-age customers find the form suitable and easy to use despite busy schedules at surgeries. Thanks to the form, they find that they manage to raise the issue of memory problems with customers/patients more often than before.
Background information
The form was developed as part of the Cerebral Health and Memory of Working-age Adults project in 2014, funded by the Finnish Slot Machine Association and based on the suggestions and needs of occupational health practitioners. The form was tested in use by nearly 50 occupational health care professionals before the completion and publication of the final version.
